Medical Assistance


In its effort to still be of service to its constituents despite the difficulties brought by COVID-19, the Senate hereby announces that requests for medical assistance can now be submitted online.

Kindly fill out the form (download SPAO form here) and, send it together with the requirements through the designated email addresses of the concerned Senators.

Requirements to be attached are as follows:

1. Personal letter to the Senator (write it in your composed email)
2. Medical Certificate or Clinical Abstract
3. Quotation of medicines / hospital bills
4. Copy of Patient’s ID and/or Representative’s ID
5. Social Case Study
• Must be a patient of the requested hospital (Government hospitals only).

Please be informed that a drop box, situated at the Senate entrance gate, is also provided for those who do not have their email addresses.
